ADULT LIABILITY WAIVER

I_______________________________________HEREBY ACKNOWLEDGE that I have applied to participate in instruction in the sport or activities, which I have initialed below, ("Sport") organized by Sports to the Maxx. I understand and I am informed that the Sport in which I wish to engage is inherently dangerous and may result in my death or injury. I shall participate in the Sport with full understanding of its dangers. Accordingly, I hereby forever waive any liability of, and release, on behalf of myself, my heirs and executors, Suzanne Maxx, Sports to the Maxx, the Cities of Malibu and Santa Monica, the County of Los Angeles, and the State of California, or any other Venues, Gyms or Clubs, Cities, Counties or States, or other governmental jurisdictions, and the State of California as well as the owners of the land and facilities where the Sports in which I participate are conducted and the heirs, successors, executors and assigns, of the foregoing released persons and entities, of and from any and all liability for my injury or death resulting from my participation in the Sport, whether resulting directly or indirectly from the negligence of any such released persons or entities or otherwise. I hereby agree that I shall fully indemnify and hold harmless each of the released persons and entities of and from all claims actions, causes of action, judgments and awards arising out of my injury, death or sickness resulting or allegedly resulting from my participation in the Sport.

I HAVE READ AND UNDERSTAND THE FOREGOING WAIVER AND RELEASE.
Nevertheless I have been advised to seek advice of an attorney to explain the foregoing provisions, and any failure to do so is my decision. I understand and agree that instruction or participation in the Sport is offered only in reliance on, and on the condition that I agree to be bound by the foregoing provisions of this Release and Waiver.

I understand and am informed that the Sport in which I wish the Minor to engage in is inherently dangerous and may result in death or injury. I agree to have the Minor participate in the Sport with full understanding of its danger. Accordingly, I hereby forever waive any liability of, and release, on behalf of the Minor and myself and the Minor's heirs and executors, Sports to the Maxx, Suzanne Maxx, and/or any other instructor associated with Sports to the Maxx, the Cities of Malibu and Santa Monica, the County of Los Angeles, and the State of California, or any other Venues, Gyms or Clubs, Cities, Counties or States, wherein the instruction takes place, as well as the owners of the land and facilities, where the Sports in which I participate are conducted and the heirs, successors, executors and assigns, of the foregoing released persons and entities, of and from any and all liability for my injury or death resulting from my participation in the Sport, whether resulting directly or indirectly from the negligence of any such released persons and entities or other wise.
